Participation in Inderia Bloom’s GLP-1 weight loss programs is subject to medical evaluation and provider discretion. Not all patients are eligible for treatment. Medical services are provided only when deemed medically appropriate by the treating provider.

An initial medical evaluation is required prior to starting treatment. This evaluation includes a review of medical history, current medications, and other relevant clinical information. The initial evaluation fee is valid for up to one (1) year unless additional evaluation is clinically indicated by the provider.

Telehealth appointments are available when clinically appropriate and are conducted in accordance with applicable state and federal laws. Patients must be physically located in a state where the treating provider is licensed at the time of the telehealth visit. Certain services, evaluations, laboratory testing, or follow-up visits may require an in-office appointment at the provider’s discretion.

Baseline laboratory testing may be required and is ordered by the medical provider based on clinical judgment. Patients are responsible for completing laboratory testing at a laboratory of their choice and for all associated laboratory fees. Laboratory costs are not included in program pricing unless explicitly stated.

All medications are prescribed based on an individual medical assessment following an in-person or telehealth evaluation. Dosing and titration schedules are determined solely by the provider based on clinical response, patient tolerance, and medical necessity.

The 3-Month Jumpstart Program includes starter dosing and early titration only, which refers to the initial dose increases commonly used when beginning GLP-1 therapy. Higher-dose escalation is not included and may require enrollment in a continuation or maintenance program.

Patients may not self-adjust medication doses without provider approval.

Medications may be dispensed in one or more shipments at the discretion of the provider and dispensing pharmacy. Once medication has been prepared, dispensed, or shipped, it cannot be returned or reused. Patients are responsible for following all prescribed dosing instructions.

Individual results vary. Inderia Bloom does not guarantee specific weight loss outcomes, timelines, or results.

Inderia Bloom reserves the right to modify, pause, or discontinue treatment if medically indicated, if a patient no longer meets eligibility requirements, or if continuation of treatment is not clinically appropriate.

Program pricing includes medical oversight and medication as outlined in the selected program. Laboratory fees, additional services, and optional add-ons are not included unless explicitly stated in writing.

Referral promotions are offered at the discretion of Inderia Bloom and may be modified or discontinued at any time without notice.

To qualify for a referral reward, referred individuals must enroll in an eligible weight loss program and complete medical intake. Referral rewards are earned only after completion of the initial 3-Month Jumpstart Program and once referral requirements are met.

Referral rewards apply to medication costs only, have no cash value, are non-transferable, and may not be combined with other promotions. Limit one referral reward per patient unless otherwise stated.

Due to the medical nature of weight loss treatments and the costs associated with medical evaluation, provider oversight, medication preparation, and pharmacy fulfillment, all program fees are non-refundable once treatment has begun.

Refunds are not provided for reasons including, but not limited to:

• Dissatisfaction with results or perceived effectiveness
• Changes in personal circumstances or financial considerations
• Voluntary discontinuation of treatment
• Finding similar medications or programs at a lower cost elsewhere
   

If a patient is determined to be medically ineligible prior to medication dispensing, a refund may be considered at the discretion of the treating physician.

If a patient is unable to continue treatment due to a documented medical contraindication or adverse reaction, eligibility for a partial refund or credit will be determined solely by the physician, based on clinical documentation and the stage of treatment.

Once medication has been dispensed or shipped, no refunds will be issued.
